What to Expect with Your First CO2 Laser Treatment
By Ronald S. Perlman, MD | October 27, 2026
If you found me by searching for a med spa near me or a trusted medspa in WASHINGTON, DC, you’re in the right place for a straight answer on what this actually looks like.
I hear nerves all the time, especially from people who’ve lived with a concern for years. One person told me I made a long-time insecurity “go away...FOREVER!” in less than an hour, and that kind of relief is exactly why I like to make the first visit feel clear and predictable.
Here’s the honest overview: expect strong numbing (usually topical, sometimes with local anesthetic), controlled resurfacing that leads to redness, swelling, and crusting, and about 7 to 14 days of visible social downtime. After that, the “settling in” phase continues. Your collagen keeps remodeling for 3 to 6 months, which is why results often keep improving long after you’re back to normal life.
Your 7-day prep plan for CO2 laser blepharoplasty-area skin
CO2 laser treatment is powerful. I don’t want you guessing in the days leading up to it. In my office, I’m known for being direct and conservative in recommendations, and that starts with prep.
Bring these details to your consult: your current skincare (especially retinoids and acids), any history of cold sores, recent tanning or heavy sun exposure, and upcoming events you can’t miss.
- Pause “active” products when I tell you to. Retinoids and harsher acids can make you more reactive. I’ll give you a stop date based on what you’re using.
- No tanning. CO2 and sun do not mix. If you’ve had a lot of sun, tell me up front so we can time it safely.
- Plan for antiviral medication if indicated. If you’re prone to cold sores, I may recommend an antiviral prescription for prevention.
- Pick your timing. Fall and winter in the Mid-Atlantic usually make strict sun avoidance easier, which is why many WASHINGTON patients schedule resurfacing then.
And yes, I also see people in for plenty of other concerns, like hair loss treatment, intense pulsed light, and “facials near me” searches. The difference with CO2 is the downtime. Timing is part of the treatment.
What happens in the room: numbing, laser passes, and keeping you comfortable
When you arrive in Tenleytown, I want your mind to settle quickly. People tell me my staff is “next level fantastic, ” and that matters because this is the kind of appointment where a calm room helps your whole body relax.
First comes numbing. Most CO2 laser resurfacing starts with topical numbing, and in some cases I’ll add local anesthetic to keep you comfortable. If you’re anxious about sensation, tell me. Pain is usually managed with numbing, oral medication when appropriate, and cooling. Sedation is something we’ll talk through during your consult so there are no surprises.
The laser part: controlled ablation, not a mystery
CO2 is an ablative resurfacing laser. That means it removes tiny columns of skin in a controlled way so your body can rebuild smoother skin and stronger collagen. It’s the reason CO2 can be so effective for deeper wrinkles and acne scars, and it’s also why you’ll look quite red and swollen right after.
Real talk on timing: one patient described a facial concern being addressed “in less than an hour.” CO2 itself varies by area, but I’m always watching the clock and your comfort. I’d rather do the right amount well than rush or overdo it.
A lot of patients are coming to me from Bethesda, Chevy Chase, Georgetown, Silver Spring, Arlington, or Takoma Park. If you’re in that group, it helps to know we’re in the Tenleytown / Friendship Heights corridor, which makes arranging a same-day ride home much easier.
“Every time I'm in his office I feel like I am with friends and family.”

The first 14 days after CO2: redness, swelling, crusting, then steady improvement
Plan for a visible healing window. Right after treatment, redness and swelling are normal. Over the next several days you’ll typically see crusting or a “sandpapery” feel as the surface heals. This is the part busy professionals and students worry about most, and it’s why I’m blunt about the timeline: give yourself 7 to 14 days before a major presentation, event, or travel.
Aftercare that actually matters
I’ll give you exact instructions, but the big themes are consistent for CO2 resurfacing:
- Gentle cleansing. Think clean hands, mild cleanser, no scrubbing.
- Frequent emollient. Keeping healing skin comfortably moist reduces cracking and the urge to pick.
- No picking. If you lift crusts early, you can prolong redness and raise the risk of marks.
- Strict sun avoidance, then daily sunscreen. Once the surface has re-epithelialized, broad-spectrum sunscreen becomes non-negotiable.
A quick makeup note: don’t rush it. I’ll tell you exactly when it’s safe to wear makeup based on how your skin is healing. Covering too early can irritate new skin and slow you down.
And then comes the part people don’t expect. Even after you look “fine” again, your skin keeps building collagen. That’s why your best results tend to keep showing up over the next few months.
Two timing tips I give almost every first-timer in WASHINGTON
Tip one: schedule around your calendar, not your impatience. If you’re tempted to squeeze CO2 in right before a wedding, conference, or photos, don’t. Give yourself the full window so you’re not stressed, tempted to pick, or trying to hide crusting with makeup too early.
Tip two: pick a season that makes sun avoidance realistic. Mid-Atlantic fall and winter often make it easier to protect healing skin. That one choice alone can make recovery feel smoother.
If you want a broader sense of what else I do in this category, I’ve put together a helpful overview on laser treatments in Tenleytown and Friendship Heights. I also break down options in laser treatment options for the Tenleytown corridor so you can compare intensity and downtime.
If you’re on the fence, here’s what I’d want you to know before you decide
People often tell me they appreciate that I listen first, then I’m honest and direct about what will and won’t help. That’s especially important with something as strong as CO2. If your goal is a subtle refresh, I’ll say so. If you need deeper resurfacing, I’ll walk you through the downtime without sugarcoating it.
One pattern I see a lot is this: someone comes in nervous, asks a long list of questions, and then becomes a regular because they feel supported before, during, and after. My team is known for being available when questions pop up, and I want you to use that. Let us know what you’re worried about. We’ll talk it through plainly.
A last note on privacy: I understand a lot of people keep results private. That’s normal, especially with facial work. I’ll focus on results that look natural and make you feel like yourself.
